Understanding Tooth Wear

What is tooth wear and what causes it?

Tooth wear — also known as tooth surface loss — is the gradual loss of the outer surface of teeth without decay. It is caused by physical forces like grinding, or chemical factors such as acidic foods and drinks. Early diagnosis is key.

Attrition Tooth-to-tooth contact

Caused by grinding or clenching (bruxism), often during sleep. Very common in people under stress or those who grind at night.

  • Flattened biting edges
  • Shortened or worn-down teeth
  • Often worsens with stress

Erosion Acid dissolving enamel

Caused by acidic foods, drinks, acid reflux, or medical conditions. Enamel gradually dissolves, exposing softer dentine beneath.

  • Thinning, translucent enamel
  • "Cupping" on chewing surfaces
  • Yellowing as dentine shows through

Abrasion Mechanical wear from outside

Most commonly caused by aggressive brushing habits, particularly with a hard-bristled toothbrush. Creates wedge-shaped defects at the gumline.

  • Notches near the gumline
  • Wedge-shaped defects
  • Sensitivity near gum margins

How It Affects You

Signs you may have tooth wear

Tooth Sensitivity

Teeth sensitivity to hot or cold foods can be an early warning sign.

Shortened Teeth

Teeth that appear shorter, flattened, or visibly worn compared to before.

Bite Changes

A noticeable difference in how teeth come together, or difficulty chewing.

Jaw Discomfort

Jaw muscle fatigue, headaches, or tenderness around the jaw joint area.

Cosmetic Concerns

Teeth looking shorter, yellower, or more translucent than they used to be.

Fracture Risk

Worn teeth become structurally weaker — more prone to cracking or chipping.

Treatment Pathway

Matched to the stage of your tooth wear

Early Stage Monitoring & Prevention

  • Diet advice to reduce acidic exposure
  • High fluoride toothpaste
  • Night guard if grinding suspected
  • Regular clinical monitoring

Moderate Stage Protection & Repair

  • Michigan splint or occlusal guard
  • Composite bonding to rebuild teeth
  • Desensitising treatments
  • Managing reflux or underlying causes

Severe Stage Full Restoration

  • Full mouth rehabilitation
  • Composite or ceramic restorations
  • Restoring lost bite height
  • Advanced occlusal planning

Clinical Case Studies

Real patients, real transformations

Case 01

Severe upper & lower wear from grinding — failing composites & reduced vertical dimension

Full mouth rehabilitation with composite restorations, restoring bite height and improving smile aesthetics across both arches.

Case 02

Severe wear with deep bite & misaligned teeth causing accelerated tooth
surface loss

Teeth first aligned with Invisalign, then brightened with professional whitening, followed by composite bonding of front teeth.

Case 03

Severe wear combined with missing teeth — requiring a multi-disciplinary approach

Complex rehabilitation rebuilding the full dentition and replacing missing teeth using a combination of restorative solutions.

Case 04

Severe upper wear treated with laboratory-made palatal composite veneers

Palatal veneers placed to restore lost upper tooth structure. Back teeth allowed to naturally erupt into the new bite — usually 6 months.

Protective Appliance

Michigan splint Protect What Matters

A custom-made hard acrylic mouthguard (also called a stabilisation or flat-plane splint) crafted to fit precisely over your teeth.

Stops Grinding Damage

Protects teeth from wear and clenching forces, especially during sleep

Relieves Jaw Joint Pain (TMJ/TMD)

Positions the jaw in a more relaxed, natural resting position

Reduces Muscle Tension

Eases facial muscle fatigue, headaches, and jaw soreness

Protects Dental Work

Safeguards crowns, veneers, composites, and other restorations

Our Restorative Services

Treatments tailored to your smile

Composite Bonding

Tooth-coloured composite resin to rebuild and reshape worn, chipped, or discoloured teeth. Minimally invasive with beautiful results.

Full Mouth Rehabilitation

Comprehensive treatment for severe wear affecting the entire mouth — restoring bite height, aesthetics, and function.

Invisalign

Clear removable aligners to straighten teeth, correcting misalignment that contributes to wear.

Veneers

Porcelain or composite shells bonded to teeth requiring minimal or no drilling of teeth to restore aesthetic function

Crowns & Bridges

A dental crown is a custom-made cap that restores a damaged tooth’s shape, strength, function, and natural appearance.

Teeth Whitening

Dental teeth whitening is a cosmetic treatment that lightens the colour of teeth by bleaching the enamel.

Implants

Dental implants are titanium tooth roots placed in the jawbone to support crowns, bridges, or replacement teeth securely.

Partial Dentures

Partial dentures are removable appliances replacing missing teeth, improving chewing, speech, and preventing nearby natural teeth from shifting.

Full Dentures

Full dentures are removable, custom-made replacements for all missing upper, lower, or both jaw teeth, restoring function and appearance.

Maryland Bridge

A Maryland bridge replaces missing front teeth using bonded wings, offering a long-term solution without drilling adjacent natural teeth.

Root Canal Treatment:

Root canal treatment (endodontic treatment) is a procedure used to save a tooth when the pulp (the soft tissue containing nerves and blood vessels inside the tooth) becomes infected or badly inflamed.

Fibre Bonded Bridge

A minimally invasive, single-visit tooth replacement option requiring no drilling, used as a conservative or temporary replacement solution.
